Failure Mode and Effects Analysis (FMEA)
The Automotive Industry Action Group (AIAG) in the QS-9000 FMEA Third Edition Manual describe an FMEA as: "...a systematic group of activities intended to: (a) recognize and evaluate the potential failure of a product/process and the effect of that failure, (b) identify actions that could eliminate or reduce the chance of the potential failure occurring, and (c) document the entire process." Simply put, Failure Mode and Effects Analysis is a tool to help you prevent product or process failures. As a preventive measure, it is best initially employed before launch of the product or process. However, it is also a "living document" which can help plan and document product and process improvements for the life of the product or process.
